What is a Fax Cover Sheet?
A fax cover sheet is the first page of a fax. It tells the receiving office who the pages are for, who sent them, how many pages to expect including the cover, and how to call back if something is missing. Faxes land on shared machines and in shared inboxes, so without a cover sheet the pages can sit unclaimed, reach the wrong person, or arrive incomplete with nobody noticing.
Healthcare, legal, insurance and government offices still use fax for referrals, prescriptions, records requests, signed forms, claims and court papers, and anyone dealing with them may be asked to fax something. Medical practices, law firms, insurance agents, title companies and payroll or HR teams send faxes as a matter of routine. Online fax services use the same cover page, so the sheet matters whether the fax leaves a machine in the office or an app on a laptop. It is also the page that makes a stack of paper traceable when someone calls to ask where it went.
The cover sheet itself is not regulated, but what travels with it may be. In US healthcare, HIPAA requires reasonable safeguards for protected health information; a cover sheet with a confidentiality statement is a common safeguard, but it does not replace confirming the number, so check your organization's policy. Lawyers have professional duties of confidentiality, and some courts that accept fax filings prescribe their own cover sheet, so check the court's rules. In the UK and the EU, data protection law governs personal data sent by fax, and Canada, Australia and India have their own privacy laws to check.
A cover sheet does not make a fax secure. Anyone standing at the receiving machine can read it and the pages behind it, which is why nothing sensitive belongs on the cover itself. A confidentiality notice does not by itself create a legal obligation on someone who receives the fax in error; its value is practical, in asking that person to call you and destroy the pages. Nor is the cover sheet proof of delivery: keep the transmission report, and call the recipient when it matters that the fax arrived complete.

How to write a Fax Cover Sheet
Fill in the recipient's details exactly
Put a named person on the To line, not just a department, with their organization, fax number and phone number. Take the fax number from a recent, trusted source — the recipient's own letter, website or a phone call — rather than an old form. Their phone number lets you check that the fax arrived complete, and lets you warn them when something sensitive is on its way.
Give your own return details
The From line carries your name, organization, fax number and direct phone number. If a page comes through blank, cut off or unreadable, the recipient needs a quick way to tell you, and the main switchboard number of a large organization is not it. If you expect a reply by fax, make sure the fax number you give is one someone actually checks.
Count every page, including the cover
Write the total number of pages including the cover sheet, and count them after the stack is final, not before you add the last attachment. A cover sheet plus a six-page contract is seven. The recipient checks the count against what came off the machine; if it is wrong, they will either chase pages that do not exist or miss ones that failed.
Add a subject and mark the action
The subject line says what the fax is in a few words: ‘Signed lease amendment, Unit 4B’ or ‘Claim form, as requested’. Then mark what you want — urgent, for review, please comment, please reply — and keep the message to what is attached and what you need, by when. Leave out diagnoses, Social Security numbers and anything else sensitive; the cover is the page everyone sees.
Add the notice and confirm the number
If the pages are confidential, add a short notice saying the fax is intended only for the named recipient and asking anyone who receives it in error to call you and destroy it. Follow your organization's policy on what may be faxed at all. Confirm the fax number before sending, warn the recipient if the content is sensitive, and keep the transmission report with your copy.
Common mistakes
- Putting a diagnosis, a Social Security number or other sensitive details in the cover message exposes them to anyone at the receiving machine, since the cover is the one page everyone reads.
- Leaving the cover sheet out of the page count, or counting before the last attachment is added, makes the recipient hunt for pages that do not exist or overlook ones that never arrived.
- Sending to a fax number copied from an old form or letterhead without confirming it can deliver confidential pages to a stranger, and no notice on the cover will undo that.
- Leaving off your direct phone number means a recipient with a blank or unreadable page has no quick way to ask for it again, so the gap goes unnoticed until someone needs the missing page.
- Treating the confidentiality notice as legal protection breeds carelessness about the number and the contents, because the notice does not by itself bind someone who receives the fax in error.
